Complete this form for each child you are registering. YOUR CHILD CANNOT BEGIN CLASSES UNLESS YOU HAVE APPLIED TO STEP UP.

2. Apply for funding.List Item 3
Step Up for Students covers your child's full tuition at PCA. You must have an Step Up application number or Award ID number to begin classes.

3. SCHOOL RECORDS
Parents of students who are enrolled in a Putnam County School District PUBLIC school MUST obtain a COMPLETE PAPER COPY of their child's school records before enrolling. Go to the school and ask for a COMPLETE COPY of your child's cumulative folder. If they deny you, remind them that the Federal FERPA law mandates they must provide you a copy. These items need to be submitted to the school office before your child will be assigned to a class.
